The following responsibilities represent the standard of conduct at the University:

Stay informed by reading communications from the University
Student Due Process Rights
The University affirms the following student rights and privileges in disciplinary proceedings for both the Complainant and Respondent
To be informed of the Student Code of Conduct and its corresponding procedures;
To petition for redress of a grievance arising from an incident which violates University policy and/or the Student Code of Conduct.
To receive notice of any alleged violation of University policy and/or breaches of the Student Code of Conduct.
To have the benefit of an opportunity to be heard by an impartial Hearing Board or Hearing Officer in addressing an allegation(s) of a violation of University policy.
To have an advisor, including an attorney, present in a hearing.
To examine evidence to be used against him or her;
To view the list of witnesses against him or her at the administrative disciplinary conference or prior to a hearing in front of a hearing board.
To request a Confidential Advisor for Title IX incidents (i.e. sexual misconduct, dating/domestic violence, stalking, harassment). The confidential advisor primarily serves to aid you in the resolution process. Confidential advisors may, as appropriate, serve as a liaison between you and the Office of Student Advocacy and Accountability and the University Police.
Disciplinary Forms and Info
This form should be completed by the accused student before their appointment date with the Conduct Authority. This form is used for both Housing and Student Advocacy and Accountability violations.

Title IX Rights & Accommodations
The University offers additional rights to student complainants for acts of violence and harassment such as sexual assault, rape, physical assault, hazing, sexual harassment, and stalking in disciplinary proceedings.
To be informed of available accommodations both on and off campus (i.e. notified of available counseling, changing academic and living arrangements);
To have a campus no contact order against the alleged student;
To have a hearing board comprised of representatives of both genders;
To have unrelated past behavior excluded from the hearing (i.e. irrelevant prior sexual history);
To testify with special accommodations (i.e. by phone, behind a screen, video, etc.);
To have no direct contact with the accused student during the hearing (i.e. questions from the accused student would be posed through the Board Chairperson and then relayed to the victim);
To provide questions to the Board Chairperson prior to or during the hearing that she/he may incorporate those into questioning the accused student;
To be informed of the names of all witnesses who will be called to give testimony;
To have an advisor, support person, or attorney (at your expense) present at all proceedings;
To request a confidential advisor- The confidential advisor primarily serves to aid you in the resolution process. Confidential advisors may, as appropriate, serve as a liaison between you and the Office of Student Advocacy and Accountability and the University Police;
To request a recess during the hearing;
To be informed of outcome of the discipline process simultaneously as the respondent;
To appeal the Hearing Board's decision as defined in Article VII, Section A of the Code of Student Conduct.
Disclosure | Transfer/Background Forms
This release represents your written consent to disclose Student Conduct Records maintained by Southeastern Louisiana University to designated parties.
This form must be completed by the appropriate official that handles student conduct administration at the college/university that you are transferring from. Your application for transfer admission will not be processed until this form is returned.
This form must be completed by the Office of Student Advocacy and Accountability at Southeastern Louisiana University before you transfer to another college/university.
Mental Health Assessment Form
This form is to be completed by a licensed mental health provider to assess readiness to return to the University in the event of a crisis intervention.
Sanction Verification Forms
To be completed by the Educational Service Project Leader to verify that service work was completed
This form is to be provided to a licensed mental health provider by the student and returned to our office. Discharge must be completed in order to meet the sanction requirement.
Students are provided a menu of educational assignments to choose from, allowing them to participate in developing their own individualized learning plan.
The purpose of this assignment is to engage the student in purposeful thought about decision-making, decision traps, and habits and identify how to make positive choices.

According to the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act), institutions which participate in Federal student financial aid programs are required to provide information to the public regarding campus safety and security policies and procedures.
File a Grievance Report (Non-Academic/Conduct Complaint)
The Grievance Reporting Form is intended for use by all students who wish to make a formal complaint about a person, policy or university process, that is not already covered by specific university procedures (e.g. Student Code of Conduct Process, Lion Intervention Network Process, Harm to Self or Others Procedures (START Process), Academic Grievance Procedures, etc.).
